Deaconess tallied three preventable errors and St. Mary`s two in Indiana`s first public round of mandatory medical error reporting, which has statewide health care improvement as its goal.
One of the most serious cases reported was the death of three babies who died after being given the wrong dosage of medication. Officials say the mistakes aren`t common, considering the overall number of medical procedures.
A preliminary list of serious adverse medical events occurring last year are posted on the indiana department of health`s web site www.in.gov/isdh the error report is part of the new federal patient safety and quality improvement act adopted by congress in 2005.
